Assessment and Assessment Summary Report

These are the standard reports issued for an initial or full assessment report. It implies an assessment against all SGP elements.

Follow-Up Assessment and Follow-Up Assessment Summary Report

These are the reports issued for an assessment geared to verify correction of previously found violations. It is not a full SGP assessment, and can only be done within 6 months of the previous audit. If not completed by this timeframe, a full re-assessment is required. A Follow-Up Assessment Summary Report only includes findings.

Assessment Summary Report Update

This report is only issued in those rare instances where verification of corrective action can be done by the service provider with pictures or documentation, in lieu of a Follow- Up Assessment. However, if a Follow- Up assessment is required to address other issues in the assessment report, the evidence should instead be verified during the Follow- Up Assessment process.

Assessment Ratings & Required Next Steps

The assessment rating, along with the facilities corrective action plan and implementation thereof, determines the appropriate next steps and their timing. Any deviation from TCCC defined timing for Follow Up Assessments and/or Re- Assessments must be approved by WA. A summary of assessment ratings is below.
