This form should be completed and submitted by Direct Import Suppliers, where Wal-Mart is responsible for the customs clearance, and also by “Disapproved” suppliers. Infor-mation for factory and supplier must be provided; agent information is required where
applicable.Please write or type legibly and email or fax to assigned merchandiser. Supplier
will be responsible for the Wal-Mart approved 3rd party audit firm’s audit fee and related costs.
