審核程序包括：審核前會(huì)議，設(shè)施巡察，查閱文件，員工面談及總結(jié)會(huì)議。
Audit procedures includes: Opening Meeting, Facility Tour, Documents review, Employee Interview and Closing
Meeting.

1. 工卡或考勤記錄 (過去十二個(gè)月)。 如果是使用電子考勤，審核員可能需要在計(jì)算機(jī)直接審閱考勤記錄，審
閱是會(huì)在企業(yè)職員協(xié)助下進(jìn)行
Timecards or Attendance Records (Last 12 Months). If electronics time card is used, the auditor may need to
review the time record directly from the computer, with the assistance from the facility staff.
2. 工資表 (過去十二個(gè)月)
Payroll Records (Last 12 months)
3. 員工花名冊(cè)及員工個(gè)人檔案（含身份證復(fù)印件）
Employee Roster and Employee Personnel Records (including I.D. card copy)
4. 勞動(dòng)合同
Labor Contract
5. 社會(huì)保險(xiǎn)收據(jù)、參保人員花名冊(cè)，當(dāng)?shù)貐⒈Ｒ笪募蚝细褡C明文件等
Social Insurance Receipts, Name List, Social Insurance Local Policy or Qualified Certificate, etc.
6. 工商營業(yè)執(zhí)照
Business Registration
7. 建筑工程消防驗(yàn)收意見書或消防備案記錄、 消防檢查報(bào)告
Construction Project Fire Safety Acceptance Document/Record or Fire Safety Inspection Certificate
8. 消防演習(xí)記錄、緊急疏散計(jì)劃及工傷記錄等
Fire Drill Record, Emergency Evacuation Plan, Work Accident Records and Work-related Injury Record
9. 特種設(shè)備注冊(cè)登記及檢驗(yàn)文件，如電梯、起重機(jī)械、機(jī)動(dòng)叉車、鍋爐及壓力容器（含氣瓶，壓力表及安全
閥）等（如有）
Special Appliance Safety Operation Certificate/ Calibration Report, such as Lift, Lifting Appliance, Forklift, Boiler
and Compressing Equipment (including Gas Cylinders, Gauge and Safety Valve) and etc., if any.
10. 特種作業(yè)人員操作證，如電工、焊接工等（如有）;
特種設(shè)備作業(yè)人員操作證，如電梯司機(jī)、叉車司機(jī)、起重機(jī)械操作工、司爐工等（如有）。
Special Operation License, such as Electrician, Welder and etc, if any;
Special Appliance Operator Certificate, such as Lift Operator, Forklift Driver, Lifting Appliance Operator, Boiler
Operator and etc., if any.

Kitchen’s Catering Service License and Cooks’ Health Certificate
12. 職業(yè)健康體檢記錄及化學(xué)品安全培訓(xùn)記錄
Occupational Health Examination Records and Chemical Safety Training Records
13. 環(huán)保文件（如建設(shè)項(xiàng)目環(huán)評(píng)、環(huán)評(píng)批復(fù)、建設(shè)項(xiàng)目竣工環(huán)境驗(yàn)收?qǐng)?bào)告，排放污染物申報(bào)登記表等)
Environmental Documents such as Environmental Impact Assessment (EIA) Report, EIA Approval, Acceptance
Check of Environmental Protection of Construction Projects and Declaration Form of Pollutants Discharged, etc.
