11a. 一般環(huán)境健康安全（General Environmental Health and Safety (EHS) ）
11.1 受核工廠應(yīng)有一套內(nèi)容包括環(huán)境衛(wèi)生安全和工作條件政策和程序的環(huán)境衛(wèi)生與安全計(jì)劃書(shū)：The facility should have a written EHS program that includes policies and procedures for environmental, health, safety (EHS) and working conditions.
11.2 受核工廠用當(dāng)?shù)卣Z(yǔ)文制訂的環(huán)境衛(wèi)生安全政策應(yīng)可供全體雇員索閱：Facility’s written EHS policies should be available to all employees in the local language.
11.3 工廠須指派一名環(huán)境衛(wèi)生與安全事務(wù)統(tǒng)籌員：An EHS coordinator should has been designated.
11.4 受核工廠須有一個(gè)負(fù)責(zé)環(huán)境衛(wèi)生與安全事務(wù)的委員會(huì)/團(tuán)隊(duì)：The facility should have an EHS committee/team.
11.5 受核工廠須有關(guān)于工作場(chǎng)所環(huán)境衛(wèi)生安全意識(shí)的推廣活動(dòng)：The facility should promote workplace EHS awareness.
11.6 所有雇員在開(kāi)始執(zhí)行新職務(wù)之前，都應(yīng)接受與新工作有關(guān)的環(huán)境衛(wèi)生安全培訓(xùn)/教育：Job-specific EHS training/education should be provided to all employees prior to starting a new job.
11.7 所有雇員都應(yīng)有機(jī)會(huì)就環(huán)境衛(wèi)生安全問(wèn)題提出意見(jiàn)/表達(dá)關(guān)注：All employees should be given the opportunity to voice EHS opinions/concerns.
11.8 應(yīng)對(duì)擬送進(jìn)受核工廠的新組件、新物料、新化學(xué)品、新機(jī)器、新產(chǎn)品實(shí)施管制，以防對(duì)環(huán)境衛(wèi)生安全有潛在損害的這些東西未經(jīng)檢驗(yàn)進(jìn)入：The controls should be in place to prevent new components, materials, chemicals, machinery, and products from entering the facility, which have not been reviewed for potential EHS hazards.
11.9 送入受核工廠的容器均應(yīng)貼附了圖文清晰的標(biāo)貼：The containers which coming into the facility should have been legibly affixed labels.
11.10 應(yīng)向承包商的人員講解作業(yè)場(chǎng)地環(huán)境衛(wèi)生安全規(guī)則，并安排他們認(rèn)識(shí)廠內(nèi)的環(huán)境：Contractor personnel should be given site EHS rules and orientation.
11.11 全體雇員均應(yīng)接受過(guò)化學(xué)品安全培訓(xùn)：The employees should have been trained on the subject of chemical safety.
11.12 所生產(chǎn)的、進(jìn)口的或使用的化學(xué)品都應(yīng)有最新的物料安全資料表（MSDS），而且都是用當(dāng)?shù)卣Z(yǔ)文編寫(xiě)的，可供全體雇員索閱：Up-to-date MSDS’s should be present for all chemical produced, imported, or used, and they should be available to all employees in the local language.
11.13 所生產(chǎn)的、進(jìn)口的和/或在作業(yè)場(chǎng)地上使用的危險(xiǎn)化學(xué)品，應(yīng)全部列入當(dāng)進(jìn)的庫(kù)存化學(xué)品目錄中：A current chemical inventory should be available for all hazardous chemicals produced, imported, and/or used at the site.
11.14 在處理和使用腐蝕性化學(xué)品或大量溶劑的地方須設(shè)置功能正常的緊急洗眼站和淋浴設(shè)備：Functional emergency eyewash station and showers should be provided where corrosive chemicals or high volumes of solvents are handled and used.
11.15 盛載著危險(xiǎn)化學(xué)品的儲(chǔ)存箱和加工箱、喉管、閥等均須附加標(biāo)簽和/或色標(biāo)：Storage and process tanks, piping, and valves containing hazardous chemicals should be labeled and/or color-coded.
11b. 健康與安全（Health and Safety）
11.16 所有已經(jīng)上報(bào)的意外事故和幾乎釀成意外事故的情況都要調(diào)查：All reported accidents and near misses should be investigated.
a) 使用調(diào)查表格調(diào)查意外事故/幾乎釀成意外事故，并完整地填寫(xiě)表格：Accident/near miss investigation form used and filled out completely.
b) 指令并鼓勵(lì)雇員報(bào)告意外事故和幾乎釀成意外事故：Employees instructed and encouraged to report accidents and near misses.
c) 跟進(jìn)糾正/預(yù)防行動(dòng)，直至這些行動(dòng)完成：Corrective/preventive actions tracked to completion.
11.17 保證上報(bào)意外事故/幾乎釀成意外事故的雇員不受到歧視：The factory should ensure employees that reporting accidents against discrimination.
11.18 穿著松身衣物、身上吊掛飾物外露和頭披長(zhǎng)發(fā)的人士一概不得接近開(kāi)動(dòng)的機(jī)器：Loose clothing, jewelry, and long hair should be prohibited near moving machinery.
11.19 全體雇員都須受過(guò)關(guān)于機(jī)器安全防護(hù)的培訓(xùn)：The employees should have been trained on the subject of machine safeguarding.
11.20 受核工廠須提供下列受用的機(jī)器安全防護(hù)裝置：操作點(diǎn)安全防護(hù)裝置、電力傳輸安全防護(hù)裝置、其它危險(xiǎn)動(dòng)件安全防護(hù)裝置、滑輪和傳動(dòng)帶安全防護(hù)裝置、風(fēng)扇葉安全防護(hù)網(wǎng)罩（網(wǎng)隙大小有一定限制）：Machine safeguards should be provided and used: Points of operation, Power transmission devices, other hazardous moving parts, Pulleys and belts, Fan blades protected with a guard and have limited openings.
11.21 應(yīng)有足夠合適的開(kāi)機(jī)/停機(jī)控制器：提供給每一名操作員、可防意外起動(dòng)、控制器附有標(biāo)貼和/或有適當(dāng)?shù)纳珮?biāo)、采用紅色掌壓式/菇狀緊急停機(jī)控制器：Starting/stopping controls should be adequate: Provided for each operator, Protected from accidental activation, Labeled and/or properly color-coded, Emergency stops of the palm/mushroom type and colored red.
11.22 凡上文沒(méi)有提及的其它機(jī)器、設(shè)備和各種固定裝置，操作狀態(tài)均須安全：Any machines, equipment, and fixtures not covered above should be in safe operating condition.
11.23 在使用涂料和化學(xué)品（例如溶劑、焊藥、粉塵）的地方須設(shè)置足夠合適的局部廢氣抽排裝置：Adequate local exhaust ventilation present where paints and chemicals (e.g., solvents, solder, dust) should be used.
11.24 任何人員暴露于危害物質(zhì)，暴露劑量均維持不超過(guò)最低安全標(biāo)準(zhǔn)：All personnel exposures should be maintained at or below safe exposure levels.
11.25 受核工廠應(yīng)有一套關(guān)于危害能源管制的程序文件：The facility should have a written procedure for the control of hazardous energy.
11.26 須為所有經(jīng)授權(quán)使用涂料/化學(xué)品的人員、受影響雇員或其它受危害能源管制程序影響的雇員提供控制程序培訓(xùn)：Training should be provided to all authorized users, affected employees, or other employees who are impacted by the hazardous energy control procedure.
11.27 須有足夠的防擾鎖具和防干擾掛簽等隔離設(shè)備可供使用：鎖具、掛簽、專組鎖箱、電箱斷路器鎖定裝置、閥門(mén)鎖定裝置、搭扣：Sufficient lockout and tagout isolation equipment should be available: Locks, Tags, Group lock box, Circuit Breaker lockouts, Valve lockouts, Hasps.
11.28 在對(duì)設(shè)備和機(jī)器進(jìn)行維修保養(yǎng)之前，首先應(yīng)把輸往這些設(shè)備/機(jī)器的能源鎖斷：The energy to equipment and machinery should be locked out prior to service or maintenance.
11.29 須完成一項(xiàng)危害因素評(píng)估工作，并保存了有關(guān)的記錄文件，確定每一項(xiàng)作業(yè)場(chǎng)需要的個(gè)人防護(hù)設(shè)備：A documented hazard assessment should has been completed which identifies the personal protective equipment (PPE) required for each operation.
11.30 雇員/訪客須獲得提供并依規(guī)定使用下列器具：眼部/面部防護(hù)器具、腳部防護(hù)器具、聽(tīng)力保護(hù)器具、手部防護(hù)器具、頭部防護(hù)器具、防護(hù)衣物、呼吸系統(tǒng)保護(hù)器具、供訪客用的個(gè)人防護(hù)器具：The following should be provided and used: Eye/face protection, Foot Protection, Hearing Protection, Hand Protection, Head Protection, Protective Clothing, Respiratory Protection, PPE Provided To Visitors.
11.31 在使用個(gè)人防護(hù)設(shè)備方面，雇員都須經(jīng)過(guò)培訓(xùn)：The employees should have been trained on the subject of personal protective equipment.
11.32 凡須穿著或佩帶個(gè)人防護(hù)設(shè)備的地方都須張貼有關(guān)的標(biāo)志：Signs should be posted in all areas where personal protective equipment must be worn.
11.33 經(jīng)授權(quán)操作起重機(jī)/吊機(jī)的人員都須經(jīng)過(guò)有關(guān)工作的安全培訓(xùn)：The authorized crane/hoist operators should be received job-specific safety training.
11.34 規(guī)定要對(duì)起重機(jī)、吊機(jī)、吊索進(jìn)行安全檢查：Inspections should be conducted on cranes, hoists, and slings.
11.35 起重機(jī)、吊索和吊機(jī)上應(yīng)清楚標(biāo)示載重容限：Capacity limits should be clearly marked on cranes, slings, and hoists.
11.36 樓梯都應(yīng)設(shè)有扶手和中高欄桿：Stairs should be provided with standard handrails and midrails.
11.37 凡開(kāi)放式樓梯、平臺(tái)和升起的地面，所有開(kāi)放邊都應(yīng)設(shè)有護(hù)欄：Open stairs, platforms, and elevated floors should be guarded by railings on open sides.
11.38 需要工人在升起的工作平臺(tái)上執(zhí)行任務(wù)時(shí)，應(yīng)只使用經(jīng)認(rèn)可的工作平臺(tái)：Only approved working platforms are used when employees are elevated to perform work.
11.39 升起的工作平臺(tái)（即吊籠）應(yīng)穩(wěn)妥地同升提設(shè)備（即鏟車、高升程裝置等等）在一起：Elevated work platforms (i.e., baskets), should be secured to the lifting device (i.e., forklift, high lift, etc).
11.40 所有梯子均須在良好的使用狀態(tài)：All ladders should be in good condition for use.
11.41 地面上和屋頂上的開(kāi)口均應(yīng)予以覆蓋或防護(hù)，以免發(fā)生人員或?qū)ο笙聣嬍录篎loor and roof openings should be covered or protected to prevent falls.
11.42 受核工廠應(yīng)有一套關(guān)于進(jìn)入密閉空間地方的程序文件：The facility should have a written procedure for entry into confined space areas.
11.43 進(jìn)入密閉空間的雇員須接受有關(guān)進(jìn)入密閉空間地方的培訓(xùn)：The employees who enter confined spaces should have been trained on the subject of entry into confined space areas.
11.44 密閉空間應(yīng)有標(biāo)識(shí)和保安措施：儲(chǔ)存缸、處理容器、設(shè)有防堤的儲(chǔ)存缸場(chǎng)、鍋爐、焗爐、地滘、壕溝、輸送管、其它密閉空間：Confined spaces must be identified and secured: Storage tanks, Process vessels, Diked tank farms, Boilers, Ovens, Silos, Underground trenches, Enclosed conveyors, Other.
11.45 經(jīng)授權(quán)操作叉車的人員須接受過(guò)有關(guān)工作的安全培訓(xùn)：The authorized forklift operator should have received forklift safety training.
11.46 貫徹對(duì)叉車的日常檢驗(yàn)和維修，以維持其良好、安全和有效狀態(tài)：Forklifts should be inspected and maintained in good, safe, working condition.
11.47 在有人流的地方都須設(shè)置“停車”標(biāo)志和鏡子，并展示了限速行車標(biāo)志：There are “Stop” signs, mirrors, and posted speed limits in areas where pedestrian traffic may be present.
11.48 工人進(jìn)入貨柜裝卸貨物之前，應(yīng)首先把掛車/貨柜穩(wěn)穩(wěn)固定，以防移動(dòng)：Trailers/containers should be secured from movement prior to entry for loading and unloading.
11c. 環(huán)境（Environmental）
11.49 有害廢棄物應(yīng)管理得當(dāng)：Hazardous waste should be managed properly.
11.50 所有有害廢棄物須在經(jīng)政府批準(zhǔn)的場(chǎng)地處置：All hazardous waste should be disposed of at a government approved site.
11.51 受核工廠須領(lǐng)有加工廠廢水排放許可證：The facility should have a process waste water discharge permit.
11.52 受核工廠須為所有氣體排放點(diǎn)在當(dāng)?shù)丨h(huán)保當(dāng)局辦妥必要的登記手續(xù)：The facility should have obtained the necessary registration from the local environmental agency for all air discharge points.
11.53 為處理生活污水，受核工廠須設(shè)有生活污水處理系統(tǒng)或領(lǐng)有排污許可證：The facility should have a septic system or discharge permit to dispose of sewage.
